, the Heart Failure Nurse Practitioner (HFNP) is an advanced practitioner in heart failure c! are in the roles of a practitioner, educator, consultant, coordinator, researcher and program implementer.
Works as the liaison between Heart Failure patients, families, nurses, physicians, administration, and other health care services.
The HFNP conducts initial evaluations with a focus on medical history, physical assessment, education, disease management, ongoing monitoring and early intervention.
The HFNP will work within the clinical protocols, recommendation and guidelines set forth by the Director of Cardiology, MD.
PRINCIPAL RESPONSIBILITIES
1.
Serves as an advanced practitioner in cardiology/HF under the supervision of a collaborating cardiologist.
Provides comprehensive assessment of complex patient, family, clinical, and community situations.
Develops and /or selects heart failure specific educational materials/programs used for patient and family education.

Collects and ! analyzes all medical information (screening, assessment, diagn! ostic testing results, risk stratification) develops diagnosis and treatment care plan for patient and family.

Maintains appropriate documentation and dictation in patient medical record.

Documents initial visit and follow up visit reports.
b.
Maintains communication with referring physicians.
c.
Makes referrals to physicians and ancillary support services when needed.
d.
Documents telephone contacts, home visits and tele-monitoring reports.
e.
Documents patient and family education.
4.
Provides oversight to clinical staff and oversees the management of HF patient care.
5.
Initiates clinical relationship during inpatient setting and coordinates continuum of care and follow up visit to HF clinic within 5 days of discharge.
6.
Conducts home visits with patients as necessary in coordination with referring physic! ians.
a.
Provides comprehensive assessment of patient to include functional, psychosocial and physical.
b.
Establishes actual and potential health problems, goals for treatment and desired outcomes.
c.
Uses advanced modalities and technologies in planning, implementing, evaluation and coordinating care activities to meet the patient and family goals and projected outcomes.
d.
Collaborates with the referring physician and HF team to identify and implement best practice for identified patient population.

The HFNP follows up on tests performed (echocardiogram, lab tests, stress test etc.) and adjusts medications/therapies accordingly.
The HFNP collaborates with the Heart Failure MD to develop a comprehensive plan for patients.
In some cases, the HFNP will collaborate with the referring MD (internist, cardiologist, primary care provider) if the patient has not been ref! erred to a heart failure specialist.
8.
To foster pro! fessional development and enhance patient care, the HFNP participates in the following:
Research trials
Committees aimed at preventing heart failure re-admissions
Continuing education
Quality improvement.
APN Council

The HFNP will take calls/patient messages from patients that they follow in APN and MD clinics during business hours.
The HFNP routinely collaborates with home care providers to ensure optimal medical care, support patient compliance, and improve continuity of care.
After office hours, coverage for HFNP patients is provided by the heart failure MD on call for shared patients, and the referring MD for patients not followed by the heart failure MDs.

MINIMUM KNOWLEDGE, SKILL AND ABILITIES REQUIRED
·
MSN or doctoral degree (DNP) from an accredited Nurse Practitioner program - Required
·
Board Certified APN in Adult, Acute Care, Family or Gerontology â" Required
·
5 years experience related to Cardiology or Heart Failure - Minimum
·
BLS/ACLS certified
·
Physical Ability to Perform Functional Requirements as Detailed
Demonstrated ability to work in a professional, multi-disciplinary t! eam as a group leader, facilitator, or participant.
Sound business judgment with a strong understanding of healthcare services, operations, finance, and patient satisfaction strategies.
Reputation as a strategic, systems thinker, and role model
Documented self-starter and team builder, recognizing and supporting Associates in achieving organizational Mission, goals and objectives.
Experience and documented abilities in the areas of cost saving, expenditure controls, revenue enhancement and efficiencies in healthcare operations.
Excellent oral and written communication skills.
